Regarding Fees & Insurance
We are committed to providing quality and affordable dental healthcare. Our fees are based on the costs of providing quality care and are the same for all patients, whether or not they have insurance coverage. As a courtesy to you, we will submit claims to your insurance company. It is your responsibility to furnish us with the necessary, correct information regarding your insurance so that we may file the claim. Your insurance may base its allowances on a fee schedule that may or may not coincide with our fees. You will be responsible for these differences in costs as well as any deductibles, co-insurance, and balances not paid by the insurance. We will do our best to attempt to determine your insurance coverage on the day of your visit. We are currently contracted with: Arise/WPS, Aurora Health Care, Delta Dental, Humana Medical, Medicare, Network Health Plan, and WEA (Wisconsin Education Association) plans, Wellpoint Dental.
The following is our office financial policy which you would receive in your new patient packet and will be required to sign before your visit. If you have any questions, our financial management team is available to answer billing and insurance questions Monday through Friday from 8:00 a.m. to 4:00 p.m. at 920-236-1680.
Financial Policy
Dr. Angell is dedicated to providing quality treatment in a compassionate and caring environment. For us to provide highly skilled care, we must be financially sound. Therefore, we require that you read, agree and sign our financial policy. Our financial team will be happy to answer any questions.
Your insurance policy is a contract between you and your insurance company. We are not responsible for knowing any benefits or coverage for your insurance plans. Insurance claims will be processed promptly. We will submit to your medical insurance first and then on to your dental insurance. We will be happy to submit a pre-treatment estimate at your request.
Our financial policy is a contract between you and this office. Consultation or office visit/examination and X-ray fees are payable in full on the day of service. Procedures performed on the day of office visit/examination are also payable in full on the service date with or without insurance.
For all other services, our financial team will provide an estimate of fees. In most cases, a percentage of the total charges will be required at the time of services. Over payments will promptly be refunded.
Extensive Treatment/Surgery
Written predetermination of benefits is required for TMJ, facial reconstructive surgery, any non-emergency procedures requiring the hospital operating rooms. This will determine your estimated out-of-pocket expense prior to surgery.
Medicare
Payment in full is due at the time of service. Medicare does not cover dental service for the care, treatment, filling, pulling or replacing of teeth.
Workman’s Compensation
Claims will be filed on your behalf. If your claim is denied you will be responsible for payment in full within 30 days of such denial.
Non-Contracted insurance companies that send payment directly you the insured are:
Blue Cross Blue Shield Insurance-Payment is due at all times of service
Non-Insured & Accident/Injury Patients-Payment in full is due at the time of service.

CHAMPUS/CHAMPVA/TRICARE
Divorced/Separated Parents
Consultation or office visit/examination is payable in full, with or without insurance, by the presenting parent. Estimate for future procedures will be given and discussed with the financial coordinator at this time. We will submit claims to your insurance on your child's behalf. The parent issuing the payment will be given the refund for any insurance reimbursement.
